Andrew Martin, M.D.

Dr. Andrew Martin is board certified in Family Medicine, Hospice, and Palliative Medicine. Dr. Martin received his Medical Degree from the University of Texas at Houston and completed his Family Practice residency at Saint Joseph’s Family Practice in Houston, Texas. In 1999, Dr. Martin received the Harold Selzman, M.D. Award for Educational Excellence. Dr. Martin founded Flagstaff Family Care Clinic in 2004 and expanded to two locations in 2009. Andrew Martin is raising two children while serving the medical needs of the Flagstaff Community.


Meghan O'Donnell, M.D.

Dr. Meghan O’Donnell graduated from the University of Colorado with a major in Kinesiology and Applied Physiology. She earned her Doctor of Medicine from Drexel University College of Medicine, Philadelphia, PA, and performed a family medicine residency at University of South Florida College of Medicine, Bayfront Medical Center in St. Petersburg, FL. Board-certified, Diplomate, American Board of Family Medicine, Dr. O’Donnell specializes in family-centered medical care including pediatrics, women’s health and chronic disease management for all. Dr. O’Donnell joined Dr's. Andrew at Flagstaff Family Care Clinic in 2008.


Michelle K. Doroz, D.O.

Dr. Michelle K. Doroz is board certified in Family Medicine. She graduated from Texas College of Osteopathic Medicine in Ft. Worth, Texas and completed her family practice residency at Phoenix Baptist Hospital in Phoenix, AZ. While in medical school, she received the National Health Service Corps Scholarship and fulfilled her commitment at Winslow Indian Health Care Center in Winslow, AZ. She then went on to work in Grand Canyon National Park at the Grand Canyon Clinic. Her interests in Family Medicine include newborn care, pediatrics, preventative care, and geriatrics. Dr. Doroz has lived in Flagstaff since 2004 and enjoys all the outdoor activities it has to offer.


Sara L. Cook, M.D.

Dr. Sara L. Cook is board certified in Family Medicine. She graduated with a major in Biochemistry from Rice University in Texas and received her medical degree from the University of Texas at Houston. She went on to family practice residency at John Peter Smith Hospital in Fort Worth, TX. She has traveled extensively overseas with her family serving in non profit work in Africa as well as India. She is excited to join the Flagstaff Family Care Clinic where she specializes in family medicine with an emphasis on pediatrics, women's health and urgent care needs. She and her husband are raising four children while serving the medical needs of the community of Flagstaff.


Christine Crowder, M.D.

Dr. Christine Crowder, M.D., is board certified in Family Medicine. She grew up in New York and graduated from Harvard University with the major of psychology and its biologic studies. She has done undergraduate research with the blind and supervised public service programs for student involvement in local communities in Boston. In medical school she enjoyed volunteering with the Philadelphia Boys and Girls Club and teaching disease prevention and health promotion for children. She graduated from the Medical College of Pennsylvania and Hahnemann University School of Medicine in Philadelphia. She then enjoyed working at Kaiser in San Francisco and relocated to Arizona to enjoy proximity to family. She appreciates her patients' trust and the caring of and learning from each one. Her greatest reward in medicine is watching her families grow through time.


Elysia Small, PA-C

Elysia Small, PA-C graduated from the University of Florida, Gainesville, FL in 2007 with a bachelor’s degree in Nutrition and a minor in Japanese Language and Literature. She has several years of experience as a Rehab Tech and CNA. She has volunteered in many capacities including as a nursing home activities volunteer and as a mentor through Big Brothers Big Sisters. She graduated in 2016 with a Master of Science in Physician Assistant Studies from A.T. Still University in Mesa, AZ and recently moved to Flagstaff, AZ to join the team at Flagstaff Family Care. She enjoys working with all ages including children.

During her free time, Elysia enjoys many outdoor activities including hiking, rock climbing, caving, and canyoneering. She also enjoys an occasional artistic endeavor and spending time with family and friends.


Laurel Steinbring, FNP-C

Laurel Steinbring, FNP-C is our new board certified nurse practitioner. She is native to Arizona. She received her Bachelors in Nursing from the University of Arizona in May of 2003. She has specialized in medical, surgical, psychiatric and school nursing over the last thirteen years. Laurel earned her Masters of Science in Nursing with a Family Nurse Practitioner Certificate at Northern Arizona University in May of 2016. Laurel enjoys the management of acute and chronic conditions. She has a passion for integrative and holistic medicine. She looks forward to serving the community at Flagstaff Family Care.


Austin Grace, PA-C

Austin Grace, PA-C, graduated from the University of Notre Dame with a dual degree in English Literature and Political Science. He recently completed his Physician Assistant education at the University of New Mexico School of Medicine in Albuquerque. Prior to PA school he worked as an Orthopedic Technician at UNM Hospital and volunteered with Albuquerque Healthcare for the Homeless. He recently relocated to Flagstaff to join Flagstaff Family Care and is looking forward to serving the medical needs of the community while also enjoying all of the outdoor recreation activities in the area with his wife and their dogs.


Patricia Bartholomew, FNP

Patricia Bartholomew is a Family Nurse Practitioner and is a graduate of Northern Arizona University for her FNP. She began her career as a Marine Biologist as a graduate of Oregon State University. She worked on board a research vessel for several years before starting her path in a medical career. She started as a phlebotomist and a diener in pathology performing autopsies in San Health Sciences University in the development of the cryotherapy treatment for Retinopathy of Prematurity with Drs. Earl Palmer and Velma Dobson. From here she began her journey into a more fulling nursing career at the Providence Medical Center in the Pacific Northwest where she began her foundation in hospice and palliative care. Today she has found the perfect balance working as a nurse practitioner in Family Practice and in Hospice and Palliative care.

When not at work Patricia spends her time with her husband and two dogs. She has several Motorcycles from sports bikes to cruisers. She loves hiking, camping and bicycle riding in the most beautiful area in the United States — Flagstaff.


Karen Damgaard, PA-C

Karen Damgaard, PA-C is a Flagstaff native. Born and raised in Flagstaff, she has always returned because she considers Flagstaff home. Karen earned a Bachelor of Fine Arts degree in Graphic Design from the University of the Pacific in Stockton, California. She later went to earn a Bachelor of Science degree in Zoology with a minor in Pre-med studies and Chemistry at Northern Arizona University. Prior to earning her Masters degree in Physician Assistant Studies at Idaho State University in Pocatello, Idaho, Karen worked for several years as a medical assistant in several private practices settings as well as a community health care facility. As a PA, Karen has spent the last 10 years serving the medical needs of the uninsured, underinsured, and privately insured people of Northern Arizona by continuing her work in a community health care center. She is excited to join the team at Flagstaff Family Care and looks forward to the challenges that lie ahead. Karen enjoys the field of family medicine because it allows her to practice medicine in a wide variety of areas while interacting with patients of all ages. Her greatest reward from practicing medicine is learning from her patients and watching them achieve optimal health.

In her spare time, Karen enjoys spending time outdoors and training for half-marathons, marathons and century bike rides. She also enjoys traveling, reading a good book and relaxing with her family, friends and dog.
